Omega Psi Phi 4th District Ohio and West Virginia
Congratulations in completing your Achievement Week Activities and Founders’ Day Banquet. There were five chapters that submitted essays (Delta Alpha, Beta Iota, Kappa Tau, Psi Omicron, and Xi Iota Iota).
The 2016 International Achievement Week Observance has now officially ended and the Form 77 is also being accepted by the Achievement Week Committee. Your submittals should cover one of the following categories listed below:
· Founders Award
· Superior Service Award
· Colonel Charles Young Military Leadership Award
· Citizen of the Year
· Undergraduate Chapter Advisor
· Graduate Chapter of the Year (Large)
· Graduate Chapter of the Year (Small)
· Undergraduate Chapter of the Year
· Omega Man of the Year (Graduate and Undergraduate)
· Basileus of the Year (Graduate and Undergraduate)
· James' Elam Lifetime Achievement Award
The nominees for the above awards must have consistently been involved at the Chapter, District and International level, excluding the Citizen of the Year. The awardees’ documentation must also show considerable community service and/or humanitarian involvement in addition to receiving community and/or humanitarian awards. The Form 77 is being accepted and the deadline date for final submission to the committee is Saturday, February 11, 2017. Please make sure your nominee information is submitted in accordance to the established deadline date. All appropriate forms are attached.
In addition, the Achievement Week Committee is encouraging Graduate Chapter to encourage their respective Undergraduate Chapter(s) to take an active part in the award’s program. The award categories for Undergraduate Chapters are as follow: Undergraduate Omega Man of the Year; Undergraduate Advisor, and Undergraduate Chapter of the Year.
